Join the Clintonville Moms’ and Dads’ Club

Please fill out this form to join the Clintonville Moms’ and Dads’ Club. Membership includes up to two parents, if both parents wish to join as part of a parental partnership. Parents are the primary members; other caregivers can participate in specific, limited activities.

We welcome parents of all types and backgrounds, and are inclusive of everyone: cis- and transgender women and men, non-binary people, agender people, and genderqueer people. If that does not align with your values, this is not the group for you.
